Improving the quality of life of people who need care and support at home

People who need care participate in determining what their care looks like in everyday life. They consult with their GP, home carer, social work and other professional or informal care providers and helpers. High-quality primary care aims to achieve dialogue: it invests in the relationship with the person who needs care and support, and takes into account the world as they experience it and their reality.

The Dr. Daniël De Coninck Fund encourages care providers and helpers – both formal and informal – to coordinate their activities more effectively. It invests in sustainable, innovative projects. These improve quality of life for people who need care and support, and enhance the capacity of care providers.
